[BlueOnyx:02579] A user account could not be created because of a system error.

Today I decided to add a new account. The creation failed with the following
error message:

"A user account could not be created because of a system error."

 

In /var/log/messages:

I enabled DEBUG in /usr/sausalito/handlers/base/user/handle_user.pl but I
can't find any debug log?

 

 

What could cause this error?
