[BlueOnyx:06907] phpMyAdmin disappears

 From time to time we see boxes where it becomes impossible to log into
phpMyAdmin.

If a user goes to the phpMyAdmin menu item from within the BlueOnyx GUI,
all they get are login screens.  Logins don't appear to actually work there.

Then users are similarly unable to access
http://server.hostname.tld/phpMyAdmin as well.

Has anybody else seen this behavior?  It's rather crippling when you
have customers wanting to access phpMyAdmin and they cannot!  :0
