[BlueOnyx:07581] Outgoing Email failure for client website ...

I received the following message as a result of a failed password request.  The server was supposed
to send a new password to the user.  The message was composed, and password generated, but this
message came to me instead of the message going out to the user.  I have seen this once before from the same user.  I have verified the email provided by them, and they and I have communicated via the email address many times over the past year.

Has anyone seen this before, and can point me to a resolution?  This is the second time I have seen a message regarding failed outgoing email.
