[BlueOnyx:07255] Box Hacked

After a shock with my hosting bill, I checked the BO Box and under "Usage
Information" in the GUI under "Network Use by IP Address" the IP address
"other" shows a huge amount of usage.

A section of log looks like:

Other - - [08/May/2011:20:07:14 +1000] "GET /input HTTP/1.1" 200 247342096
"" ""
Other - - [08/May/2011:20:07:14 +1000] "GET /output HTTP/1.1" 200 1833095173
"" ""
Other - - [08/May/2011:20:07:14 +1000] "GET /forward HTTP/1.1" 200 0 "" ""

What does this mean?
Why is there no IP address associated with the data transfers?
How do I find out what has been hacked and how do I fix it?
