Yes, it has always needed a connection to MySQL to store things like user
settings or address books.

Depends on the version of RoundCube. Starting with RoundCube v0.7.0 it's a
WebApp and the setup and the install of the MySQL database is handled through
the WebApp installer.
So please do this:
1.) Go to "Server Managemeng" / "Network Services" / "MySQL". Make sure the
MySQL server is enabled and that a password is set for MySQL user "root". The
field " MySQL Status:" should read: "MySQL connection possible with this
settings."
2.) Go to "Server Management" / "Web Applications", "Setup SQL". When you
click on the button "Test MySQL Credentials" it should tell you that a
connection to MySQL is possible with those settings. If not, enter the right
access details on that page.
3.) Once that is done, it should be possible for RoundCube to connect to
MySQL. If not, then uninstall and reinstall the RoundCube deployments to the
individual Vsites through "Server Management" / "Web Applications", "WebApp
Installer", which shows a list of all sites. Just click on the pen icon next
to each site to handle the uninstall and reinstall.
IF this is an older RoundCube (older than v0.7.0), then do this:
1.) Go to "Server Managemeng" / "Network Services" / "MySQL". Make sure the
MySQL server is enabled and that a password is set for MySQL user "root". The
field " MySQL Status:" should read: "MySQL connection possible with this
settings."
2.) Restart CCEd: /etc/init.d/cced.init restart
