Chris,
I have also several other 5108R Systems and no other have this error.
For Blueonyx the regular PHP is running

For websites the solarspeed php version is running.
In adm_error Log I see the additional information
[Mon Dec 03 09:03:04 2012] [error] [client 1.2.3.4] PHP Fatal error: include() [<a href='function.include'>function.include</a>]: Cannot redeclare class uiextension in /usr/sausalito/ui/extensions/modify.User/subdomains.php on line 6, referer: https://www.wallenfels.com:81/base/user/userList.php?group=site1
Do you have any ideas what I can check to solve this issue?

I just tried on 2 different 5108R boxes, both fully updated, and was not able to duplicate the failure.
Is this a "stock" server? ie: regular PHP, not upgraded outside of
BlueOnyx, etc? When I have seen the "Cannot redeclare class
uiextension" before, that has usually been on a system that has had PHP "tweaked".
BTW: One of our employees is in Frankfurt now. She says it's lovely.
