Thanks for taking the time to respond. Here's the results of the tests:
I have added mail.mtbe.co.uk to the Email Server Aliases via GUI.
"service sendmail restart" reported no problems.
/user/sbin/sendmail -bv postmaster at mtbe.co.uk returns:
rewrite: excessive recursion (max 50), ruleset canonify
rewrite: excessive recursion (max 50), ruleset parse
postmaster at mtbe.co.uk... User unknown
This happens no matter what postmaster is changed to - even a valid account
name (as opposed to an alias) fails.
more /etc/mail/local-host-names | grep mtbe.co.uk returns the expected hosts
- mail, www and mtbe.co.uk as well as a test we added today.
more /etc/mail/virtusertable | grep mtbe.co.uk returns:

How do I flush the DNS cache on BX? I've restarted named but can't see nscd
anywhere.

DNS now looks good.
A couple of quick tests:
Check you have "mtbe.co.uk" in the GUI "Mailer Server Aliases" and *also*
"mail.mtbe.co.uk".
Save
[shell]
service sendmail restart
/usr/sbin/sendmail -bv postmaster at mtbe.co.uk
No luck, then check:
[shell]
more /etc/mail/local-host-names | grep mtbe.co.uk
-This should have 3 entries like: mtbe.co.uk, mail.mtbe.co.uk,www.mtbe.co.uk

A couple of use recently had an issue with building a domain name with
"test.domain.tld"
then changing the hostname from "test" to "www".
This appeared to have worked but after tests we found the hostname value are
appended into
the "/etc/mail/access" file, so it find the old record (i.e. "test.") and
stops processing!
There should be no records in here for your domain:

Also I see the priority is '0' is that valid?
Maybe change and see if it helps.
0 is a perfectly valid MX pref. (16bit unsigned int per RFCs) - one might
argue if its smart to use it but it's valid. But I fully I second your
statement regarding CNAMEs, the record for www is quite sure the culprit
here.
