[BlueOnyx:13179] Sendmail excessive recursion on a customer server

We are trying to assist a customer with email trouble on his domain, and 
not having much luck.   The issue revolves around the dreaded old 5.0.0 
rewrite: excessive recursion.    We're usually able to address these 
issues quickly enough by paying some attention to the DNS.   This time, 
not so much.

Using the information in this post:

we have verified:
1: DNS is pointed to the customer server
2: Domain contains a valid MX record, which is directed to a valid A record
3: Domain "Email Server Alias" includes just the domain name


Advice/ideas are welcome.

some interesting diagnostics:
# sendmail -bv test-user at forgruene.com
