What is the "md126" and "md127" from?  It looks like you added a disk 
into /dev/sdb that had pre-existing RAID data on it.  That's going to be 
a problem, and you'll need to unmount anything that has been mounted 
from /dev/sdb and then fdisk and repartition.   Then you can add the new 
disk into the RAID, but you'll need to walk it through manually.

Be very careful, of course, when you're working with partitions.  The 
old woodworking adage "measure twice, cut once" should apply here!
