[BlueOnyx:19479] Cannot Add Users Chorizo Error

Hi, 

We have a severe problem on an Upgraded Virtual Blueonyx 5107,

When clicking the add user Button we get this error.

Does anyone have any pointers, last week we added a user withour problems
