[BlueOnyx:19047] New RAID drive gives a “Kernel panic - not syncing - Attempted to kill init”

Hi,

I'd appreciate some help if anyone has ran into this issue before.  I 
have a blueonyx server with raid1.  One of the drives was complaining of 
SMART errors so proactively I decided to replace the drive.  When I do, 
the server freezes during the boot and displays “Kernel panic - not 
syncing - Attempted to kill init”.  If I put the failing drive back in 
the server boots normally again.  I think I have replaced drives 
previously without issue.  Should I be using mdadm to remove the failing 
drive first?
