[BlueOnyx:26646] error creating a user

When I try to create an admin user for a vsite I am getting this error

I blew away the site, recreated it, and then tried again. Same errors

Checked my user name and password for funky characters. None

Restarted CCed

Same error

Since we are talking errors.....
For some time now when I use phpmyadmin it throws an error message (see
attached capture).

Everything seems to work ok after I click the message away.

Don't like errors :-)
