It was in fact the /etc/passwd.lock
I did run the dbrecover commands and restarted cced multiple times.
Thanks for the suggestion though

Have you ran
   /etc/init.d/dbrecover stop
   /etc/init.d/dbrecover start
   /etc/init.d/cced.init restart
Then try to create a user.
If the mail hashes are OK, it may be a problem with Berkley database
