When booting off the CD, the Linux kernel on the CD is called with the 
parameter "ks=cdrom:/ks/kickstart.cfg". Which tells the installer to load the 
kickstart config from the CD-ROM. This kickstart file defines what is 
installed and where it is supposed to be installed to.

So the probable cause for an incomplete install here is that your Xen box got 
confused about the CD-ROM device. This problem can happen if there are 
multiple (real or virtual) CD-ROM drives. The system may boot off the primary 
CD-ROM, but then - seeing multiple CD-ROM drives - dissagrees about which one 
is the primary one that holds the installation media with the kickstart 
script.
