What i did:

I had some manual configuration in:
/etc/httpd/conf.d so the didn't know the new settings

So I added this to my config files 
php_admin_value safe_mode_include_dir /usr/sausalito/configs/php/
php_admin_flag mail.add_x_header On
php_admin_value sendmail_path /usr/sausalito/sbin/phpsendmail
php_admin_value auto_prepend_file
/usr/sausalito/configs/php/set_php_headers.php
php_admin_value safe_mode_protected_env_vars LD_LIBRARY_PATH

phpmyadmin was also not working
/etc/httpd/conf.d/phpMyAdmin.conf
Was missing 
php_admin_value open_basedir /usr/sausalito/configs/php/
so I added it to the file
php_admin_value open_basedir
/usr/share/phpMyAdmin/:/tmp/:/var/lib/php/session/:/etc/phpMyAdmin/:/usr/sau
salito/configs/php/

so for the sites that is not working check the settings

Even my roundcube mail down. any idea how to fix this as the phone keep on

We are still experiencing a problem on 5106R
no php sites are working.
We have done the 'yum clean all ; yum -y update'
server has been rebooted

still php errors
