Yep,

That sounds like a box that had the BlueOnyx install CD left in it when it was rebooted. 

A few years back, one of my customers left a BX CD in his big virtual host after loading a new VM with BX.  When his NOC went down to a power distribution fault, the big virtual host reloaded and wiped 20+ VMs turning it into a big BX server.

Shortly after learning of that, Michael put in the requirement to hit enter before a default install will start - to prevent such accidents.  But if your Colo guys hit enter at the opening screen - you've got a newly installed BX server and all the old stuff is gone.

Hope you had backups....
