This looks a lot like the issue that was raised earlier and resolved in 
"[BlueOnyx:18013] Re: Outbound Email from VPS", where Michael Stauber 
advised the following:

Check if the file /etc/hostname exists in that 5209R VPS. It might be
missing.

If so: Then that's related to an issue that was fixed in the lastest
5209R OS template. The older 5209R templates were missing a specific
startup script that OpenVZ uses to set the hostname of the VPS during
startup.

So do a "yum update" on the node to make sure you have the 5209R OS
template named blueonyx-5209R-CentOS7-x86_64-BX02-20150629 installed.
That will also get you the previously missing startup script for OpenVZ.

Then restart the existing 5209R VPS and it'll set the hostname for it
correctly in *all* the right places. Including in /etc/hostname

But while you are at it: Also check /etc/hosts to see if all the info in
it is correct.
