Try this as root from the shell:

/sbin/service pam_abl restart

That will delete the PAM_ABL and will recreate an empty database shortly
thereafter.

The "ccewrap" lines you reported: That's normal. That happens when the
GUI uses the CCEWrap mechanism to execute certain sanitized and limited
commands on the shell.
