    This is create via guy, or CMU import? Either way, I’d be looking at /var/log/messages to see what handler is causing the error and starting there… Many of the handlers contain debug options - just changing the debug option might give you enough to get a clue or two

This is via the GUI ... 

Following the CMU import I noticed that not all the users had migrated across.
When I try  recreating a user via the GUI:

GUI shows UNKNOWN ERROR DURING CREATE

/var/log/messages:
