What does /var/log/messages report when you try to create one of those 
two users?

I presume it will show an error while executing one transaction or 
another during the attempted user creation. The point where it errors 
out might indicate to us what the culprit might be.

This could be a leftover email alias in CODB, a failure during the 
rebuilding of the email server mapfiles or a couple of other things we 
haven't covered so far.
