[BlueOnyx:00554] Site admin cannot create user - permission denied

A customer noticed that a site admin is not able to create a user and
gets a message that "Permission for CREATE is denied."   I have
duplicated this error with the following steps:

1) Log into BlueOnyx as site admin
2) Click to Add new user
3) Correctly fill in information and click "Save"

Output in red error at bottom of BlueOnyx interface:
Sorry, an internal server error occurred. Permission for CREATE is
denied. Please visit http://BlueOnyx.it/ for technical info.

Output in /var/log/admserv/adm_error:
