Some of these steps may not be necessary, but this worked for me with a
similar problem

cd /etc/mail
make clean
make
/etc/init.d/dbrecover stop
/etc/init.d/dbrecover start
service sendmail restart
/etc/init.d/cced.init restart

Then try to create a user
