[root at 5106r ]# rpm -qa|grep phpMyAdmin
phpMyAdmin-2.11.9.4-3.centos5

Apparently your server is tied into an unsupported YUM repository, from which 
it installed a newer (and different) phpMyAdmin than the one that BlueOnyx 
5106R usually uses.

Solution: Disable or remove the unsupported third party YUM repositories. 
Remove phpMyAdmin via "rpm -e phpMyAdmin --nodeps" and then "yum install 
phpMyAdmin" to get back the one that you're supposed to have.
