Here is what I suggest:
1) Remove the add-in card with extra Ethernet slots.
2) Eliminate the extension you are using. It's not all that clear in
the picture but it looks to me like telco grade. Just to eliminate one more
variable.
3) Re-install BX from scratch. RHEL6 based distros track eth port
numbers via their mac addresses and if you don't do this you may have your
system not believe your eth0 is one of the remaining two ports.
