[BlueOnyx:10173] Roundcube database error: connection failed

We have a customer that installed the Solarspeed RoundCube webmail 
client.  They are receiving this error when they go to their 
domain/roundcube to log in:

DATABASE ERROR: CONNECTION FAILED!

Unable to connect to the database!
Please contact your server-administrator.


I was unaware that a database was required or that further setup is 
needed beyond installing the PKG.  Does anybody have an idea as to what 
to do next?
