Thank you for sending me that screenshot.

No, actually the form is as intended. The makers of WHMCS allow for up
to 24 custom form fields on that form.

When you add formfields to it in the module, the first formfield is
placed in the left column. The second formfield in the right column. The
third in the left column again ... and so on.

That's why "username" is on the right side (as it's the second item) and
"password" is then on the left, as it's the third item.

I didn't like the way the formatted this, but there is not much that I
can do about it.

As for the field named "WHM Package Name": It was named that way in the
official developer module template, so I left it deliberately named that
way. I just assumed that the WMCS guys know how their stuff is named.
But nice to see their not immune to typos in the UI as well. ;-)
