Andy,

Wow, feel for you.  Just had that on a 5210R box yesterday.
Tried the disk_restorequotas.pl and swatch to no avail.
A full power off reboot of the server seems (prayers) to
have solved it for us.  In our case it was only one user out
of 15 on the site and both site and all users were way, way
under their allowed quota.  (During this none of the other
sites or users on this box were having any issues)...
